MIDOC — Medical Certificate Workflow Automation
A custom Wix form workflow that turns submitted information into a formatted medical certificate PDF and delivers the document automatically by email.
MIDOC needed a straightforward online flow to collect required information, generate consistently formatted certificate documents and deliver them to users. The workflow required conditional fields, input validation and careful handling of personal information.
Created a responsive onboarding form with conditional logic and real-time validation. Connected submissions to backend Velo logic that populates a structured PDF certificate template and sends the generated document as an email attachment. Tested the complete submission, generation and delivery flow across devices and scenarios.
Custom Wix forms and JavaScript validation connected to Velo backend processing, dynamic PDF template population and automated email delivery with attachments.
Users can complete one online form and receive a generated certificate document by email, reducing manual document preparation and delivery steps while keeping the experience consistent.
